REGISTRATION INFORMATION
In order to attend the party, each student must complete both the Registration and Medical Release Form.
It's easy to register - please follow the steps/options below.
1) Download, complete and sign the Registration Form and Medical Release Form

2) Return the signed Medical Release Form and Registration Form one of these ways:
- Email to [email protected].
- Mail to WHS Grad Night PO Box 14, Wilsonville, OR 97070
- Drop off at the WHS Main Office, "Grad Night Mailbox"
- Pay online via the "Donate" link below
- Mail a check along with the signed Medical Release Form and Registration Form
- Drop off a check along with the signed Medical Release Form and Registration form to the WHS Main Office
